Functional recovery review. Exercise program progression, return-to-work guidance, and identification of any issues requiring follow-up with a local specialist.
Comprehensive recovery review. Blood panels where applicable, imaging review if needed, and confirmation that recovery is progressing within expected parameters.
One-year outcome assessment. Full recovery review and documentation of outcomes — for your records and for your home physician. The coordination program formally closes at this stage.
